A medication log,
to tick off each dose.
This is a blank medication log to print or download as a PDF, in A4 or US Letter. Each row records one dose: the date, the time, the medicine, the amount, a tick when it’s taken and the initials of whoever gave it. It suits a family or a helper sharing the care of one person.

A blank sheet from alongwell.app. AlongWell keeps your family’s medicines and readings on your phone, free on iPhone and Android.
How to use it
- Print the log. Download the PDF in A4 or US Letter, or print this page.
- Fill a row per dose. Write the date, the time, the medicine and the amount the prescription gives, then tick it when it’s taken.
- Initial it. Whoever gave the dose adds their initials, so everyone sharing the care can see it was done.
Who it’s for
A medication log helps when more than one person looks after someone’s medicines: family members taking turns, or a helper at home. Everyone can see what was given, when, and by whom, and nothing gets given twice.
Writing the amount
Write the amount exactly as the prescription or the label gives it. The log records what was given; it doesn’t say what to give.
Keeping it on your phone instead
AlongWell keeps this record for you: every dose marked taken shows who marked it, and a family member can mark their own dose by replying on WhatsApp. It’s free on iPhone and Android.
