ready to print.
This is a blank blood pressure log to print or download as a PDF, in A4 or US Letter. Each row has room for the date, the time, both blood pressure numbers, the pulse and a note. No target ranges are printed on it, so it suits anyone: your doctor is the one to say what your numbers mean.

A blank sheet from alongwell.app. AlongWell keeps your family’s medicines and readings on your phone, free on iPhone and Android.
How to use it
- Print the log. Download the PDF in A4 or US Letter, or print this page.
- Write each reading. Copy the date, the time, the top and bottom numbers and the pulse from your monitor’s screen.
- Bring it along. Take the sheet to your appointments so your doctor sees every reading.
What goes in each column
Copy the numbers exactly as your monitor shows them. The two blood pressure numbers are in mmHg and the pulse is in beats per minute. Use the notes for anything you’d want to remember later: which arm, whether it was before or after a medicine, or how you were feeling.
Why there are no ranges on it
Many printed logs carry colour bands for “normal” and “high”. This one leaves them out. What a reading means depends on the person, so the sheet only keeps the numbers and leaves the meaning to your doctor.
Keeping it on your phone instead
AlongWell keeps the same log on your phone for each person in your family, with a chart over a week, a month or a year. It’s free on iPhone and Android.
