A blank sheet from alongwell.app. AlongWell keeps your family’s medicines and readings on your phone, free on iPhone and Android.
How to use it
- Print the log. Download the PDF in A4 or US Letter, or print this page.
- Write each reading. Note the date, the time, the number from your meter and whether it was fasting, after a meal or random.
- Bring it along. Take the sheet to your appointments so your doctor sees every reading.
What goes in each column
Copy the number exactly as your meter shows it, and write the unit once at the top. Mark each reading as fasting, after a meal or random, because the same number can mean different things at different times. The notes column is for anything else: what you ate, a missed medicine, or how you felt.
Why there are no ranges on it
The sheet keeps the numbers and leaves out “normal” bands. What a reading means depends on the person and on when it was taken, so that’s for your doctor to say.
Keeping it on your phone instead
AlongWell keeps blood sugar readings for each family member in mg/dL or mmol/L, marked fasting, after a meal or random. It’s free on iPhone and Android.
